To obtain the Police Clearance Certificate from Ukraine the following documents should be submitted. 

- copy of the first page (with personal data) of international passport; copy of the passport page with Ukrainian visa (if available) and/or copy of the Temporary Residence Permit in Ukraine (if available) or exact dates (period) of residence in Ukraine mentioned in the application form.
